Services: The Interdisciplinary Team including a nurse, social worker, therapist, dietitian, pharmacist, program aides and volunteers working with the patient, family and attending physician to provide the following services:
personalized treatment plan
health care assessments
physical, occupational and speech therapies
social services
psychiatric and psychological services
activities and socialization
personal care assistance
lunch, snacks and nutritional counseling
door to door transportation
Participant Eligibility: Services for frail elderly and functionally impaired adults with all of the following:
A Medical condition requiring treatment or rehab services prescribed by a physician.
Physical/mental condition that impairs activities of daily living but are not so severe as to require 24 hour institutional care.
Reasonable expectation that preventive services will maintain or improve the present level of functioning.
A high potential for further deterioration and probably institutionalization if ADHC were not available.
Maximum one person assist required for transfers and ambulation.
Referrals: May be made by patient and family, physician or other health care and social service professionals.
Payment:
- MediCal Reimbursement for eligible participants - Sliding scale for private pay
Adult Day Care Adult Day Care is a drop in program designed to promote socialization and stimulation for participants and to support caregivers.
Services Include:
activities and socialization
personal care assistance
lunch
caregiver respite
door to door transportation
visits may be regularly scheduled or as needed for respite
Participant Eligibility:
No nursing care required.
Maximum one person assist for transfers and ambulation.
Ability to participate in activities.
Referrals: May be made by individual, family member, social service or health care professional. Doctors orders are not required for the day care program.
Payment:
- Private pay: $25/day
Alzheimer’s Day Car Resource Center: Funded in part by PSA 2 Area Agency on Aging.
